Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and Complaint Investigation. A photo was received showing an infant in their crib with a bottle An infant was placed on their stomach to sleep, an infant had a bottle in their crib, and sleep checks were not conducted every 15 minutes. An infant was secluded in their crib for around 15-20 minutes to prevent undesired behaviors. An educator in infant B did not wash their hands, the children's hands, or clean the changing table between uses. An educator walked out of the infant B classroom, leaving 3 infants unattended. One of the times the educator walked out was while 3 infants were in their highchairs, eating.

See Below for Detailed Information Observations: Licensing viewed the video feed from the date of the incident that took place on 06/03/2021, where a child was left unattended in their classroom without adult supervision. This occurred during transition. 1240-04-01-.11 SUPERVISION Corrective Action: Agency has been retrained and will follow rules and do a face to name roll during any transition. Observation: It was observed on 06/04/2021 that director and upper management had knowledge of a child being left unattended in a classroom. Both parties agreed that they knew of the incident but failed to report it. 1240-04-01-.09 INCIDENT REPORTING. Corrective Action: The childcare agency retrained the staff on name to face transition procedure and developed protocol for management to conduct daily checks during transitions as they occur to ensure proper procedure is being followed.

Corrective Action: Agency will have all employees cleared with a background check prior to starting work.

See Below for Detailed Information Observation: Licensing verified a complaint that a child was bit numerous times and only one incident report was completed and shared with the parents. 1240-04-01-.09 INCIDENT REPORTING. Corrective Action: The agency will train staff regarding the incident reporting rules and insure that the reports are completed as required. Licensing also made a referral for TA, additional training for staff in this area. Observation: Licensing verified a complaint that many daily reports sent to a two-year old child's parents had inadequate/inaccurate information. 1240-04-01-.08 RECORD KEEPING. Corrective Action: The agency will train it's educators regarding he proper way to enter daily reports and insure that this is done accurately in the future. Licensing also made a referral for TA for the agency to receive additional training. Observation: Licensing verified a complaint that a child's diaper was not changed and child cleaned promptly resulting in blisters. In addition, daily reports completed for this child concerning diapering were sometimes inaccurate (child reported diapered twice before she arrived), lapses of more than 4 hours in-between diaper changes, as well as diaper changes only documented once or twice a day. 1240-04-01-.12 HEALTH AND SAFETY. Corrective Action: The agency will train staff regarding child care diapering requirements and ensure that children are diapered properly. Licensing also requested TA for additional training for staff. Observation: Licensing verified a complaint that a child with a milk allergy (posted in the classroom) was served and ingested milk on at least two occasions, documented in the child's daily reports. Therefore, it was determined that the mealtime supervision plan was not followed for this child. 1240-04-01-.11 SUPERVISION. Corrective Action: Licensing made a referral for infant and toddler staff, as well as administrative staff to receive training regarding allergies and following their written mealtime supervision plan. .

See Below for Detailed Information Observation: While investigating a complaint of ratio violations, licensing observed a grouping violation in a toddler room. The allowable adult:child ratio in this classroom is 2:12, with a maximum of 12 children in the classroom. This classroom had 14 toddlers present. 1240-04-01-.22 SPECIFIC REQUIREMENTS FOR CHILD CARE CENTERS. Corrective Action:: The agency reported it will insure that the group size is not exceeded in any classrooms. Observation: On 10/7/20, licensing validated a complaint that alleged that ratios were out of compliance in several classrooms. Two toddler classrooms were found out of compliance. One classroom maintained a 1:7 ratio and the other a 2:14 ratio, the allowable adult:child ratio for this age group is 1:6 or 2:12; A two-year-old classroom also exceeded the adult:child ratio for this age group as 1 educator cared for:11 children. The allowable adult:child ratio is 1:7. 1240-04-01-.22 SPECIFIC REQUIREMENTS FOR CHILD CARE CENTERS Corrective Action: The agency reported that they will correct the ratio violation by insuring enough staff are scheduled for each classroom. Licensing will conduct additional training for the administrative staff. Observation: Licensing observed that some children were not in their assigned classrooms and had been moved to other classrooms (shuffled) due to staff shortage. 1240-04-01-.22 SPECIFIC REQUIREMENTS FOR CHILD CARE CENTERS: Corrective Action: The agency reported it will insure that children stay in their assigned classrooms throughout the day.
