Director did not notify the Department the same day that a complaint from a parent came in about an educator and the educator was fired. Director did not notify DCS of this complaint which would be considered suspected verbal abuse. Director did not report in the DHS portal an incident where a phsycial altercation occurred in the infant room between two employees and police were called. This phsycial altercation was also not reported to DCS.

Also educator got up with children to assist in washing their hands and left one child at the table with food and her back was turned to him.
