During the serious incident investigation, LC checked the children's files for an official incident report for each child involved in the incident. No incident report was completed that meets the DHS Rules and Regulations requirements; however, the Director did have a document of an email that was sent to the Parents of the children involved, and an in-person meeting was held.

The Agency self-reported that two children, aged 17 months and 22 months, walked out of the playground gate and down the sidewalk to the front of the childcare center on Tuesday, September 2, 2025. The educator in charge of their care was unaware that the children were gone and left unsupervised. The Director saw the children in front of the building and immediately retrieved the children and brought them safely back into the childcare center.
