Summary of a Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint investigation visit. See Below for Detailed Information Observation: A ten month old was placed in a high chair to feed her a bottle but because she is still an infant she needs to be held even if she can sit up. 1240-04-03-.11 Food (2) (k) Corrective Action: Director will make sure staff understands that children who are 12mo and younger are held when drinking a bottle. Observation: Preschool I had a 1:20 ratio with age’s 40-63months and it should have been 1:13 or 2:22 they were over 7. Preschool II had a ratio of 1:13 25-47mo should have been 1:9 or 2:18 so they were over 4. Toddler I was 2:13, 19-25mo and should have been 1:6 or 2:12 they were over one. Toddler II ratio was 1:9 they had 27-35mo and should have been 1:7 or 2:14 they were 2 over. 1240-04-03-.06 Supervision (3) (d) 1-2 Corrective Action: Director went through and checked all appropriate ages groups to make sure children were in the correct class rooms based upon age. She also stated that as a company will make sure enough staff is hired and on hand at all times. This will take care of the supervision issue. Will daily check ages grouping? Birthdays and ratio charts are still posted in each classroom. Will also make sure teachers know correct age group. *Observation: The playground has some protrusion hazards that need to be fixed. 1240-04-3.08 Equipment (1) (d) Corrective Action: Make sure equipment is accurate/up to date and without hazards. *Observation: Children did not always have access to all materials. And some classes did not have a sufficient amount. 1240-04-03.08 Equipment (2) (b) 1-3 (c) Corrective Action: Director will make sure that teachers understand that children must have access to all centers and toys in the classroom. Observation: Playground equipment was not spaces out far enough. And there was not enough accessible resilient surfacing. 1240-04-03-.08 Equipment (3) (c) 1-2 (e) *Observation: children were viewed placing toys in their mouth and not being cleaned. 1240-04-03.08 Equipment (1) (e) Corrective Action: Director will make sure that children are better supervised at all time and that toys are placed in a yuk bucket when they have been in the children’s mouth. Observation: During assessment during TV time children weren’t allowed to do anything else but watch TV. 1240-04-03.09 Program (2) (k) Corrective Action: Director will let teachers know that children should have other options during TV time. Observation: During assessment the schedule was not being followed. 1240-04-03.09 Program (1) (a) Corrective Action: Make sure teachers know and follow schedule as closely as possible and to allow free play throughout the day. Observation: During assessment there was not a balance between child’s choice and adult directed activities. Observation: During assessment it appeared that children were stationed longer than 30 minutes in a crib. 1240-04-03-.09 Program (1) (g) Corrective Action: Director will make sure children are moved around more frequently. Observation: Children weren’t able to go out on this day. So indoor play only occurred for the girls and not the boys. 1240-04-03-.09 Program (3) (a) (7) (d) Corrective Action: Director will make sure that children will get free play. Observation: the infants didn’t always have access to all their toys. Some only had access to a few at a time. They should have floor time and activities that develop physical, social, language and cognitive skills and language experience with adults daily. 1240-04-03.09 Program (7) (e) (f) Corrective Action: Director will make sure that children have access to all materials and more language experience.
