Summary of complaint, follow up, and safety plan served Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint, follow up, and safety plan served . If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. LC watched video footage from 5/14/25 and observed one educator helping kids get onto cots and one educator was sitting with children at the table while they ate. The educator then got up and walked into the bathroom and came back and was cleaning the tables while one child was still eating.

Summary of Complaint investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and Complaint investigation. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. While watching video footage from Monday 5/12/25 at lunch a educator in the 4-5 year old room was observed helping children in the bathroom, and the other was observed picking up and stacking chairs while children were still eating. This is a lapse of meal time supervision as a teacher should be sitting with the children while they eat.

During an unannounced visit on 8/22/24 video footage was reviewed for 8/20/24 which showed the 2 year old classroom at a ratio of 1:8 for 24 minutes. The ratio for this age group should be 1:7
