During an unannounced complaint visit on 10/18/24, LC Walker observed video footage of two (2) educators of the 2-2.5-year-old classroom, leaving a child (2 year old) unsupervised in the classroom alone for approximately six (6) minutes while going to another classroom to have Music and Movement and not conducting Name to Face roll call properly. In a classroom there was bleach water up in a cabinet not locked up, this is a QRIS indicator not a rule violation. During an unannounced complaint visit on 08/01/24, LC Walker observed video footage of the educator of the 12–18-month-old classroom, leaving a child (15-month-old) unsupervised in the hallway strapped in a buggy for approximately four (4) minutes. During an unannounced complaint visit on 08/01/24, LC Walker observed video footage of a 15 month old child left in the hallway outside the classroom for approximately four (4) minutes due to the educator not conducting Name to Face roll call before entering the classroom. Management is responsible for training and evaluating staff to ensure supervision procedures are being followed. During an unannounced complaint visit on 08/01/24, LC Walker observed video footage of the educator of the 12–18-month-old classroom, leaving a child (15-month-old) unsupervised in the hallway strapped in a buggy for approximately four (4) minutes.

LC Walker also observed the educator use a harsh tone during this time.

During an unannounced complaint visit on 04/29/24, LC Walker observed video footage of an educator in the 3–4-year-old classroom use a harsh method when going to get a child out of the bathroom before lunch time. She was observed using a harsh tone with the child as well as getting up and going to get the child with her hands on the child's back pushing her to go sit down.
