See Below for Detailed Information Observation: A new staff member did not have proof/documentation of before you begin training and shaken baby syndrome/abusive head trauma as required. 1240-04-01-06 Staff Corrective Plan: The new staff member will be required to complete the trainings. Observation: There was no proof/documentation of a current training regarding procedures to report child abuse and neglect for an educator as required. 1240-04-01-10 Duty to Report Child Abuse and Neglect Corrective Plan: The educator will receive training regarding procedures to report child abuse and neglect for an educator. Observation: There was no proof of education for two new educators as required. 1240-04-01-08 Record Keeping Corrective Plan: The educators will be required to provide proof of education.
