Ratio in the Two & Three classroom was not met. Ratio was 1:14., Should have been 2:14 Agency was using the Naptime Supervision rule. However, chidlren in the classroom were ages, 24-35 months. Naptime relaxed ratio can only be used with children 31 months and above.

The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint or safety plan]. delivery. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Educator was observed forcing children to remain on mats during naptime. Educator was observed using inappropriate discipline techniques during naptime. Agency failed to document and report an incident as required. Agency failed to report an incident as required.

Violation: 1240-04-01-22, Specfic Requiements for Child Care Centers (1) Required Adult: Child Ratios (d) Assignment of Children to Groups (3) Infant was grouped with three and four year old chidlren until the Infant educator arrived for her shift. Observed: Infant was placed with three's & four year old children until the Infant room educator arrived for her shift. 1240-04-01-22, Specific requirements for Child Care Centers, (1) Required Adult Child Ratios, (d) Assignment of Children to Groups (3) Infants shall have a separate space and shall never be grouped with children older than thirty (30) months of age.
