Summary of Safety Plan Monitoring and Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations. Technical Assistance Provided: 1. LC discussed five year re-prints. Email notifications were sent out. On-site director stated that she is receiving the notifications. However some notifications were not received for staff who were printed in May of 2018. LC stated that even if a notification is not received, they still need to be re-printed. 2. LC discussed the law regarding the child abuse coordinator and back-up coordinator. No violations were received

Summary of Put on Notice (PON) delivery, Safety Plan Monitoring, and Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations. Technical Assistance Provided: 1. LC reviewed the change report with the site director. Site director will add the days of the week in the box for hours of operation. 2. LC reviewed the two plans of corrective action (POCAs) with the site director. The two POCAs address the immediate action that was taken for both violations, as well as how the agency will prevent the violation from happening in the future. The LC advised the agency that they add additional prevention measures to the POCA to address when the agency will notify parents that new medication needs to be brought in and by what date the medication must be brought in. The LC advised the agency that they add additional prevention measures to the POCA to address the root problem of reporting incidents to parents/guardians on the same day of the incident. 3. LC emailed and reviewed the Put on Notice (PON) letter with the directors. LC emailed the agency a parent signature sheet (HS-3432). 4. LC reviewed the safety plan supervision logs and scanned copies of the logs. 5. LC provided a list of educators who are coming up on their five year re-prints. 6. LC recommended that educators sing songs with children or count or participate in fingerplays, while they are in line for the bathroom. 7. LC reached out to her FS and then advised the directors to add "optional snack" to their menu for the one classroom that serves a snack due to a child's doctor's orders requiring a snack. Today a family brought in food for lunch. The LC recommended that the agency update the menu prior to families bringing in items for lunch. Going forward, the agency will serve all items posted on their menus and will not substitute items posted on the menu with the items a family brings in. If a family brings in food, the food will be optional to children, and it will be listed on the menu as optional. LC reminded the directors of the rule "meal and snack substitutions shall be noted on the menu in advance." No violations were received

Observation: During a complaint investigation it was found that an educator used inappropriate discipline with children by yelling, using a harsh tone and picking children up inappropriately by their arms. 1240-04-01-.15 Program, Language and Literacy Development Corrective Action: all staff (including management, administrators, auxiliary and educators) have received training in developmentally appropriate practices, and appropriate behavior management and guidance.

Once the schedule is made, a copy will be sent to the LC. The teachers submit a field trip request form at least two weeks in advance to the on-site director. On-site director will send approved field trips as they come in. 3. LC reminded on-site director that many staff are due for their five year re-prints before May of 2018. The clearance must be completed prior to the most recent clearance. 4. On-site director will complete a change report to update the agency's hours of operation. Violations were received and include: 1.
