See Below for Detailed Information OBSERVATION: On 1-8-19 it was observed by the Assessor that hot dogs were not appropriately cut up to prevent a choking hazard. 1240-04-01-.13 (2) (b) Food-Choking Prevention CORRECTIVE ACTION: Agency will now be quartering hot dogs to serve to non school aged children. OBSERVATION: On 1-17-19 it was observed that the agency was not within the appropriate adult:child ratios. 1240-04-01-.21 (1) (b) Required Adult:Child Ratios-With one educator present at the agency there were also 8 children with 7 of those children being under three years old, requiring an additional educator by rule. CORRECTIVE ACTION: A second educator came back to the agency which put the agency back within the appropriate adult:child ratio.
