The incident was not reported to DHS on date of incident. The director reported that nothing had happened so she was unaware that she needed to report the incident. Name to face recognition was not used when returning from playground No incident report was completed for the parent to sign Child was left on playground unsupervised on 9-15-2023 for approximately 5 minutes. The child was 3 years old. He was found on the playground by the day care center staff as his mother pulled up into the parking lot. This was not reported to DHS until October 11, 2023.
