Once it was brought to the agency's attention, the agency proceeded to investigate prior to and without notifying the proper entities. 1240-04-01-.10 DUTY TO REPORT CHILD ABUSE AND NEGLECT Corrective Action: Director states that she will report all major incidents to DHS and DCS in the future. Agency completed training through CCR&R. Observation: A couple of months ago, a child exited their classroom to the hallway without educators being aware the child was gone until time had passed. 1240-04-01-.11 SUPERVISION Corrective Action: Agency completed training through CCR&R on supervision. Observation: Agency did not provide a parent documentation of an incident or have them sign documentation of an incident the same day the incident occurred, and the report was not filed in the child’s record at the agency. 1240-04-01-.09 INCIDENT REPORTING Corrective Action: Agency completed training through CCR&R on incident reporting.
