Observations: On 5.9.18 during an unannounced visit PE observed open S hooks on the large pre-school playground on both swing-sets. Also observed was a lack of the required 6" of resilient surface around the fall zones of the swing-sets 1240-04-03-.08 Equipment for Children. Plan of Corrective Action: On 5.14.18 Ms. Pence emailed to say all swings will be removed from the playground.
