Fire inspection was done March 4, 2026 and March 11, 2026 both times disapproved.

It also included Complaint and Safety Plan Visit. During the investigation, it was determined that the educator was out of the required adult‑to‑child ratio of 1:9 when the incident occurred yesterday in the 18–36‑month classroom.

It also included a Complaint, Safety Plan, and Technical Assistance If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. A child was found walking around in the parking lot of a strip mall after leaving the daycare because the doors were unlocked at the time of incident until a Krystal's Restaurant employee found him and called the police. The front door was broken due to someone trying to break into the agency. LC did not observe an incident report in the child's file. LC observed the attendance logs with several children not signed out June 2, 2025, June 5, 2025, and June 9, 2025.
