Summary of QRIS Monitoring and Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and scoring of the QRIS and Complaint Investigation. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. LC observed the educator on her telephone while holding an infant that was crying. The educator was not monitoring the children that were asleep. LC did not observe a mealtime supervision plan in two of the classrooms. LC provided TA. LC did not observe a playground supervision plan posted in any of the classroom, nor by the door that's used for children to exit to enter the playground. LC observed the playground rules posted inside the fence on the playground. LC provided TA. LC observed the educator on her cell phone while holding a child that was crying and agitated. The educator failed to respond/attend to the child. Licensing Consultant (LC) conducted an unannounced visit on November 19, 2025. During the visit, LC observed the toddler educator holding a child that was crying and agitated, while on her cell phone (scrolling). The educator neglected to attend to the child and was not monitoring the children that were asleep during nap time.
