On April 8, 2025 (LC), Licensing Consultant observed the three(3)-5(five) year old classroom out of ratio. The classroom had a ratio of 1:17; the correct ratio for this age group is 1:13. On April 8, 2025, (LC) Licensing Consultant, Naomi Jones, observed that a five-month(5)-old child's daily activity sheets were inconsistent with feeding for several days. The parent of the child expredded concerns when she reviewed the daily activity sheets

During an unannounced QRIS visit on May 9, 2025, Licensing Consultant (LC) Suziet Smith and Field Supervisor(FS) Remell Coleman, observed that the mealtime supervision plan was not posted in classrooms 2 and 3 (toddlers) & classroom 4 (preschoolers).
