Summary of complaint investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint visit. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. During an unannounced complaint investigation visit made on 1-23-25, the Licensing Consultant observed the adult:child ratio to be 1:8 in the 12 - 30 months class with the one educator. There was no other educator in the class with her during my observation.

See Below for Detailed Information Observation: PE observed A lock was placed on the doorknob in the 2-year-old class and gate was still up in the Toddler classroom; after PE asked the agency to take it down because its blocking the exit. All these locks on the door and the gate is a fire hazard. 1240-04-01-.16 Physical Facilities Corrective Action: Provider will ensure that no gates are placed in front of exit doors and no locks are placed on the door knobs.
