Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint investigation visit. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. On November 11, 2025, Licensing Consultant (LC) viewed video footage of a reported incident that happened at the center on October 24, 2025, that involved a child causing harm to another child while the educator was in the classroom with her back turned to the incident. The educator failed to turn around for a significant amount of time, even while an attempt to notify her was made by one of the students. Licensing Consultant (LC) conducted an unannounced visit on November 3, 2025 at 10:30 am. During the visit, LC viewed files of the children named in the complaint. While viewing the files. LC noticed that an incident report was missing for one of the mentioned incidents, in one of the children's file. The incident report was only in one of the children's file.

The Infant/Toddler classroom is on multi-age grouping where the ratio is 1:5. LC observed 1:6 in the classroom as they were transitioning from lunch to nap time. The educator reported that she has 6 in attendance and the LC counted 6 children.
LC was unable to access the records of the children. The onsite director reported that she does not have access to the files that are locked in the main office. LC was unable to view certificates to compare due to the records being unavailable upon request.

LC was unable to verify all subsidy children based on sign in/out sheets. DIrector reported that she is unable to print all of the certificates from the portal. LC only viewed 40 certificates. There were 39 children on the roster for today. LC requested last two weeks sign in/out sheets. The director stated that she will have to have the owner print them from the procare system. LC was unable to verify all certificate children based on sign in/out sheets.

LC viewed the sign in and out sheets that are printed from the procare system. There is no signature on the printed sign in/out sheet form. LC inquired about the parents signature and the director reported that the parents use their thumb print as proof that the child has been signed in. LC requested that the director create a line on the dace to name sheet for parents to sign the child in an out. Per the rules, the sign in/out sheets must contain space for printed name and signature.
