On November 25, 2025, the parent reported to the director that the infant educator pulled his child by the arm and tapped her bottom. The director did not report the incident to the Complaint Hotline On November 26, 2025, when checking the educator's file, it was noted that the educator started working on November 11, 2025, and does not have a physical on file at the agency. On November 25, 2025, the director was notified of an incident in the infant room, but it was not reported.

Upon arrival at 8 am, it was noted that the preschool room had ages 2.5 years to 10 years with one educator and seventeen children. One new educators file did not have the following: Documentation that staff have been screened and/or tested for all communicable diseases as required by the Department of Health before having contact with children.

This is a quality indicator only, it was noted that children all had to sit at the table and wait to use the restroom when they came in from the playground. Children were up from their seats, redirection being used for behavior. To receive credit children could be staggered in to use the restroom if there was extra staff. Another way to help with this is by giving children some table activities to work on while they wait their turn to use the restroom.

This is a licensing requirement at all times. However, the agency was out of ratio on October 11, 2024, which resulted in a violation. At this time the violation was noted for the 10/11/2024 visit and no the agency will not be receiving quality points which is required for this indicator.

It was noted on the playground several broken plastic containers that children were using to play and store materials in which need to be removed. In the toddler classroom, it was noted a few broken toys on the playground such as the car. The materials that were kept out of reach were noted in the restroom areas were wet wipes which have to be locked up to receive the quality points also in this category.

The agency completed a monthly fire drill in March but did not complete a fire drill in April according to the director. Monthly fire drills must be conducted according to the fire codes. Also the operating hours were not posted in the child care facility.
The preschool classroom was out of ratio. Ages 3 years to 5 years there were 14 children with one educator. Ratio was 1:14 and should be 1:13. The max group size for this classroom is fourteen children. When fourteen children are present there must be two educators at all times.
