(b) Child care agencies shall develop and follow a written playground supervision plan that includes: 6. Roll call before leaving classroom and upon arrival at playground and prior to leaving playground and upon arrival in classroom. Observation: The classroom educator reported that on the day in question, she did not call roll or count the children as they transitioned from the restroom to going outdoors. Plan of corrective action: A POCA was emailed to the director on 8-12-21 and was completed, signed, and returned to LC Hancock on 8-16-21. The agency is also on a safety plan, which states that all staff will receive CCR&R training on supervision, specifically supervision during transition times. In addition, at least one extra staff person will observed classrooms during transition times to ensure that face-to-name roll call is being done as they move from one area to another. Rule: 1240-04-01-.06 STAFF. (3) General Child Care Agency Staff Qualifications. (c) A staff member shall be designated to be in charge of the child care agency in the absence of the primary educator/director and the name of the person in authority shall be posted. Such person shall be familiar with child care agency policies and procedures. Observation: No director or person in charge was posted. Mrs. Burnett is the new director. Although the LC was told that Mrs. Burnett would be taking over it was not clear when her start date as director would be. No one was clear during the visit on 8/12/2021 when the date was. It was confirmed to the LC on 8/13/2021 that her take over date was 7/26/2021. Plan of corrective action: A POCA was emailed to the director on 8-13-21 and was completed, signed and returned to LC Hancock on 8-16-21.

Observation: The agency failed to complete a February fire drill as required. 1240-04-01-.16 Physical Facilities Corrective Action: The agency plans to conduct a fire drill early next week and will document it. In the future, fire drills will be planned for the first week of the month if possible to insure that it is conducted monthly as required.

Observation: An educator was working without a cleared background check as required. 1240-04-01-.07 Criminal Background Check and State Registry Review Procedures Corrective Action: The agency immediately sent the employee home pending receipt of the cleared for work letter. The letter was received later that same afternoon and the employee can return to work effective 3/6/20. The violation has been corrected.

Observation: The agency failed to conduct a fire drill in the month of September as required. The director is new to this position and reported that she did not realize that fire drills must be completed monthly. 1240-04-01-.16 PHYSICAL FACILITIES Corrective Action: The agency conducted a fire drill on 10/24/19 and the director sent licensing verification that the drill was completed.
