Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and investigating complaint #PCS1002728. Technical Assistance Provided: 1. Having an individual clipboard in each classroom for quick referenc e of attendance for each class 2. Completing incident reporting the same day as an incident occurs 3. Making sure playground is safe and playground supervision plan is accurate and pertinent The gate leading from the playground to the parking lot is in poor repair. No classroom had every child signed in during the visit. A parent/guardian was not notified of a serious incident. There was a broken gate on the playground, which allowed a child to escape. A child escaped the playground area and was in the parking lot/near the road. A passerby stopped and got the child and took her back to the daycare.

Observation: Child care licensing staff observed grouping violations in the preschool classroom. One of the two staff in the pre-k room was also responsible for food service for the entire daycare which is a violation if you are to be counted in adult/child ratios. 1240-04-03-.06 SUPERVISION Corrective Action: Agency agreed to have a separate person on duty for food service only. Also, a substitute will be used if necessary. Technical assistance given at visit.
