Summary of QRIS Monitoring Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and scoring of the QRIS. During the Teacher-Child Observation in the Infant A classroom the educators talked to the children rarely. When they did it was mostly for routine care. Educators in the Infant A classroom did not actively engage with the children with language or imitation during the classroom observation. In the Infant A classroom the educators did not provide rich social interactions such as smiling, talking, touching, singing, calling child by name, and engaging in eye contact. See Below for Detailed Information Observation: During the complaint investigation, staff verbalized they suspected abuse neglect dealing with another staff member but did not report as a mandated reporter. This was failure to report. 1240-04-01-.10 DUTY TO REPORT CHILD ABUSE AND NEGLECT. Corrective Action: TA was given to the director and all staff interviewed to remind them they are mandated reporters and must report any suspected abuse or neglect.

See Below for Detailed Information Observation: There was no transition plan for children moving. Parents and staff were no all aware of the transition. 1240-04-01-.08 Record Keeping Corrective Action: Agency was sent a sample transition letter and plans to start using written communication. Observation: There was one infant observed on his stomach with a blanket bunched up under his face and chin. He was awake but had been asleep for 7-8 minutes according to the lead educator. The blanket and child were removed from the crib. 1240-04-01-.11 Supervision Corrective Action: The blanket was immediately removed. Agency will receive new training from CCR&R. Observation: Educator did not perform duties satisfactorily in the toddler classroom, when children were allowed to continue to climb on and over a toddler sofa that was turned upside down presumably resulting in the injury of a child. 1240-04-01-.06 Staff Corrective Action: Agency will receive training from CCR&R on classroom management.
