See Below for Detailed Information Observation: Child care licensing validated a complaint received that on 5/24/18 a two year old child was accidentally left outside on the playground unsupervised for a period of approximately 15 minutes. Rule: 1240-4-3-.06 SUPERVISION Corrective Action- The agency director has reviewed their playground supervision procedures and trained staff. A safety plan will be implemented to insure that all staff will receive additional supervision training by child care licensing staff and Child Care Resource and Referral staff.
