See Below for Detailed Information Observation: On 6/12/19, the agency self-reported that a four year old child left the agency unsupervised and unnoticed during nap time and walked to his grandmother's house nearby. He was not injured. 1240-04-01-.11 SUPERVISION. Corrective Action: The agency added alarms to the front and back doors and was receptive to supervision training by the licensing PE and CCR&R staff.
