Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations, a complaint visit, and a safety plan served. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Parents were not made aware of an incident that occurred at the center. The name to face roll call was not completed prior to transitioning to the playground. A child was left unattended while at the center.

See Below for Detailed Information Observation: LC Brooks noted from reviewing video footage toddler on playground crawling on concrete walk way on the the toddler playground without shoes or socks. 1240-04-01-.11 SUPERVISION. Plan of Corrective Action: Director updated the policy requiring all children to have footwear appropriate for outdoor play. Educators will ensure children have on footwear prior to outdoor play. Observation: LC Brooks observed video of children continuously crawling on pavement on he playground without shoes or socks . 1240-01-04.15 Program Language and Literacy Development Corrective Action: Agency has modified center policy requiring appropriate footwear for active/outdoor play. Parents/Guardians were notified of change in policy. CCR&R will provide technical assistance for safe outdoor surfacing for the age and developmental level of the children. Observation: LC Brooks determined a toddler age child was on the playground on 8/2/2021 crawling and pulling up on a fence without shoes or socks. The child was primarily observed on the concrete pathway on the playground. 140-04-01-.12 HEALTH AND SAFETY . Corrective Action: Agency has revised agency policy requiring all children to have appropriate footwear for active/outdoor play. CCR&R will be providing technical assistance for parents/guardians were notified of change in policy. CCR&R will provide technical assistance for safe outdoor surfacing for the age and developmental level of the children

See Below for Detailed Information Observation: children and staff being yelled at, spoken to in harsh tones. 1240-01-04.15 Program Language and Literacy Development - Behavior Management and Guidance Corrective Action: CCR&R coach will provide conscious discipline training to all staff Observation: child was found outside of playground fence behind trash bins, educators unaware of her whereabouts. 1240-04-01-.11 Supervision Corrective Action: CCR&R coach will provide onsite playground management and supervision training to all staff. New gate latches (requiring adult to push from top) have been installed. A new system for tracking children is in place, each group has a printed roster (laminated) and marker.
