Also, both of these children did not belong in this room. One should have gone to Ms. Jamie¿s two year old room and the other two year old to Ms. Willette¿s room. You did correct this issue immediately after I called your attention to it. The licensing rule [1240-4-3.06(3)(d)(chart 1)] definition states: The adult:child ratio for toddler age twelve months to thirty months is 1:6 with a maximum group size of 12. While I was at the child care center today, I observed snack time. I observed three different classrooms today. In two of the rooms, children did not have their hands washed before snack time. In these same two rooms, the tables were not cleaned or sanitized before snack was served. In the infant room, an infant was placed in a high chair. The high chair tray was not cleaned or sanitized before the child was placed in this high chair to drink a bottle of juice. The tray was visible dirty. In the infant room, the daily talk sheets for the six infants present were not being completed in a timely manner. My visit was at 2pm. No naps had been recorded for any of the six infants in care on this day. There were also very few bottle feedings or diaper changes documented also. While the teachers told me that they had been changing diapers, feeding children, checking children when they napped for SIDS, etc., they had not been timely documenting these actions. It could be very difficult to remember how much a child ate and at what time they ate, when diapers were changed and when naps were taken if they are not documented within a reasonable time frame. Documentation of these daily routines is very important to ensure that children of this age have the care they need to thrive, grow and develop. The licensing rule [1240-4-3-.05(8)(b)(1)] definition states: Additional information for infants, toddlers and all non-verbal children shall be recorded and shared with parents daily as follows: the time and amount of feeding, any incidence of excessive spitting up, toileting and/or times of diaper changes, sleep patterns, and developmental progress. In Kerris¿ three year old room, Kerris placed the cookies on a napkin on the table while the children were sitting down. The children began to eat immediately. She then turned her back and began to place the straws into the juice boxes at the counter. Kenisha was in the room also. She was walking around the two tables and then went to help Kerris place the straws in the juice boxes and then had her back turned to the children also. After they passed out the juice boxes to all of the children, they both did sit at the tables with the children. However, they did not sit with the children while they were eating cookies. These teachers also did not have direct supervision while children were already eating cookies and they were finishing putting out juice boxes. In Willette¿s two and a half and three year old room, she and Kelly put out cookies in front of the children and then placed the juice boxes in front of the children. While this did not take long, children had already begun to eat and where without direct supervision until both teachers sat at each of the two tables. The licensing rule [1240-4-3.06(1)(a)(1)] definition states: The management of the agency shall maintain a system that enables all children in the agency¿s care to receive a level of supervision of their status and activities that is appropriate to their age and their developmental, physical and mental status so as ensure their health and safety and that allows agency personnel to know the whereabouts of each child in their care. As we discussed, you need to have a specific place for your night shift children to go to when they arrive for care in the afternoons. The night shift children should not be moved or shuffled around during their time in your care. They need a stable teacher and stable space while at your child care center. You have twelve children who are enrolled for night shift. You should have a concrete plan of when these children arrive and where they will go for care. You may need to ensure that you have addressed policies concerning arrival for your second shift children in your center handbook. This will help parents know what you expect in regards to when they can arrive, what they are expected to do if they should be late or early for care, etc. Also, as we discussed, the two one year old teachers, Ms. Ryann and Ms. Keisha, should have notified you immediately when this mother arrived with two children for care and that this situation placed them over ratio and grouping sizes. Because they did not notify you, it appears that this situation may occur often and that the teachers are accustomed to this occurring. Please refer to page 45 of the Licensure Rules for Child Care Centers, 1240-4-3-.10 (14) (b) which states: (14) Diapering. (b) For the protection of children and adults, the Centers for Disease Control guidelines for handwashing and diapering procedures shall be followed. A caregiver¿s comb with a long, sharp pick was on a shelf within the children¿s reach. Please refer to page 45 of the Licensing Rules for Child Care Centers, 1240-4-3-.10 (13) (e) which states: (13) Prohibited Practices and Products. (e) Kitchen knives and other potentially dangerous utensils or tools shall be secured so that they are inaccessible to children. A caregiver¿s purse was also observed on a changing table. Please refer to page 45 of the Licensure Rules for Child Care Centers, 1240-4-3-.10 (13) (f) which states: (13) Prohibited Practices and Products. (f) Staff¿s personal belongings (such as, but not limited to, contents of purses, backpacks, coat pockets, diaper bags, etc.) shall be inaccessible to children at all times. The week¿s menu was not posted. Please refer to page 50 of the Licensure Rules for Child Care Centers, 1240-4-3-.11 (1) (e) which states: (1) Nutritional Needs. (e) In order for parents to be aware of the food their children are receiving the week¿s menus shall be planned and posted by the first day of each week and remain posted throughout the week. Infants¿ bottles were not labeled. Please refer to page 51 of the Licensure Rules for Child Care Centers, 1240-4-3-.11 (2) (h) 1. which states: (2) Meal Service. (h) Formula and Food Brought from Home. 1. All formulas and food brought from home shall be labeled with the child¿s name. A child was in a crib with a bottle. Please refer to page 52 of the Licensure Rules for Child Care Centers, 1240-4-3-.11 (2) (k) and (m) which states: (3) Meal Service. (k) Infants shall be held while being fed as long as they are unable to sit in a high chair, an infant seat, or at the table. (m) Bottles shall not be propped, and a child shall not be given a bottle while lying flat. Snacks were sitting on a changing table. Please refer to page 53 of the Licensure Rules for Child Care Centers, 1240-4-3-.11 (3) (f) which states: (3) Food Storage: (f) All food shall be protected from contamination during storage, preparation, transportation, and serving. Two rooms were out of compliance with the adult:child ratio. Ms. Willette had 10 children ages 2 ½ to 3 years and Ms. Debra had 16 children ages 3 to school-age. You stated Ms Debra¿s room was supposed to be 3 and 4 year old children. Please refer to page 22 of the Licensure Rules for Child Care Centers, Rule-1240-3-.06 (3) (a) and (d) Chart 2 which states: (3) Required Adult:Child Ratios. (a) The adult:child ratios shall be maintained by the child care agency while the children are indoors and on the playground. (d) Adult:Child Ratio Charts Chart 2 shows the correct adult:child ratio is 1:9 for a group of children ages 2 ½ to 3 years old. It also shows the correct ratio for a group of children ages 3 to 4 years 1:13. Children were not assigned to a specific group and were waiting to be told which room to go to. Please refer to page 21 of the Licensure Rules for Child Care Centers, Rule 1240-4-3-.06 (2) (a) and (e) which states: (2) Assignment of Children to Groups. (a) Each child must be on roll in a defined group and assigned to that group with a specific caregiver(s). (e) In order to assure the continuity of care for children and their caregivers, the children shall be kept with the same group throughout the day and shall not be moved, shuffled, or promoted to a new group until required based upon the developmental needs of the child.
