Infants shall be touched by an educator every fifteen (15) minutes in order to check breathing, body temperature and position; Observation: During the visit on 7/18/19, the educator was asked to perform a SIDS check on a sleeping infant after 16 minutes lapsed. The educator admitted that a SIDS check had not been performed for 36 minutes total. PE observed the sign-in sheet and only (8) eight students were signed in on the daily sign in sheet. PE informed the teacher (3) three students weren’t signed in, she stated that the three students are transitioning students from the 3-4 year old class. PE observed the 3-4 year old class and the teacher stated that she has (9) nine present now and (3) three transitioning across the hall. PE observed the (3) three students were signed in, to the classroom in which they are enrolled. PE was provided with student’s files and proper documentation wasn’t available in the files.
