Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and Complaint If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Incident Report was not completed immediately upon incident nor was a copy provided to the parent on the same day. The incident was also not reported to DHS or DCS on the same day of occurrence. After an incident occurred at the agency on 2/04/2025, the agency conducted its own investigation without reporting to DHS or DCS.
