See Below for Detailed Information Observation: During the program assessment, the assessor stated she observed a child in the 12-30 month old class choking on a sandwich. The assessor removed the food from the child's mouth. The sandwich had not been cut into small pieces as required to prevent choking. 1240-04-01-.13 FOOD AND FOOD SERVICE Corrective Action: Staff received individual training on cutting up foods to prevent choking and a handout on preventing choking has been posted in each classroom. .

See Below for Detailed Information Observation: A child was injured while on the playground. The staff responsible for supervising and protecting the children did not see the injury occur and were not following the child care agency's Playground Supervision Plan. 1240-04-03-.06 SUPERVISION. Corrective Action: The child care director will train staff on the child care's current Playground Supervision Plan. The training entitled "Active Supervision of Children" by the Child Care Referral & Resource Center will be scheduled and attended by all staff by June 30, 2018. Observation: Agency did not report when a child was injured on the playground from an unknown source. 1240-04-03-10 HEALTH AND SAFETY. Corrective Action: The agency will train all staff in reporting abuse/neglect by June 30, 2018. Observation: On May 4, 2018 three classrooms were using the playground at the same time. An educator admitted that while her classroom was on the playground, she went inside the center with a sick child leaving her classroom to be supervised by the other teachers which caused the adult:child ratios to be out of compliance. 1240-04-03-.06 SUPERVISION Corrective Action: Child care will train staff on the child care's playground supervision plan which states "All teachers are to be on the playground while their students are outside. If there is a need for you to come inside, call inside the building for someone to come cover your class".
