Summary of [complaint investigation and safety plan delivery ] Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint investigation and safety plan delivery. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Management of the agency did not ensure a serious incident report was documented and reported to DHS and DCS as required. Proper staffing, grouping, ratios, and supervision was not ensured by management. The children were improperly grouped per ratio and grouping chart. On March 5, 2024 and incident of a child born on 9.27.24 (who left the class and agency unsupervised) was not reported to the Department of Children's Services or the Department of Human Services Child Care Licensing as required by licening rules. No incident report was written. On March 5, 2024 a child born 9.27.21 was able to leave the classroom and exit the agency unseen by three classrooom staff.
