Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Educators were observed picking children up by one or both arms and forcefully placing them onto their mats during nap. After the educator changed the diaper, the educator did not wash the one-year-old child's hands or her own hands. Children were observed being picked up by their arms or one arm. The LC observed a video from 4/29/2026, and on that video, a child was observed eating at the table, and the three educators in the classroom were busy cleaning, changing diapers, and laying out mats for naptime. None of the edcuators were directly supervising the child while the child was eating. Items labeled keep out of reach of children were located on open shelves in two different classrooms.

During the monitoring visit on 8/13/2024, Licensing Consultant (LC) Tina Gosson completed a staff file review. One staff member did not have CPR training verification in her file. The director explained that this educator had not completed the training, but had signed up for it. The employee was hired on May 8, 2024.
