Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and . complaint investigation. During today's unannounced visit, the Licensing Consultant observed the caregiver supervising seven children. The caregiver's CPR/First Aid certification expired in May 2025. See Below for Detailed Information Observation 2: During Assessment, caregiver was observed leaving three children alone while eating a snack at the table while she prepared a bottle in the kitchen. 1240-04-01-.11 SUPERVISION (2) Meal and Snack Time Supervision. (a) During meal and snack time, staff shall maintain direct supervision of children between six (6) weeks and five (5) years of age and maintain supervision of children between six (6) and nine (9) years of age. Compliance Statement: In order to ensure compliance, the agency will ensure that children are never left alone during meals or snack time. Observation : During Assessment, caregiver was observed not conducting SIDS checks every fifteen minutes. 1240-04-01-.11 SUPERVISION (6) Safe Sleep Supervision Procedures 9. Infants shall be touched by an educator every fifteen (15) minutes in order to check breathing, body temperature and position. Compliance Statement: In order to ensure compliance, the agency will ensure that staff are following safe sleep practices and conducting SIDS checks by touching sleeping children every fifteen minutes.
