See Below for Detailed Information Observation: On February 20, 2018 the PE confirmed that on February 6, 2018, during the announced assessment visit, the Primary Caregiver told the Assessor that the children only go outside if the temperature is forty degrees or above. The provider told the Program Evaluator the children were taken outside as long as the temperature was above thirty five degrees (35 degrees). 1240-04-01-.05 PROGRAM. Corrective Action: A plan of corrective action was left with the provider to complete and return to the PE. PE reviewed with provider that the children should go outside daily unless inclement weather (rain or less than 32 degrees including windchill). The provider noted she understood and would come into immediate compliance with this licensing rule. Observation: On February 20, 2018 the PE confirmed that on February 6, 2018, during the announced assessment visit the caregiver reusing the same rag to wipe the table during the two step cleaning process. 1240-04-01-.08 FOOD. Corrective Action: A plan of corrective action was left with the provider to complete. The PE provided technical assistance and reviewed with the provider that a separate cloth should be used so as not to re-contaminate the eating surface. Observation: On February 20, 2018 the PE confirmed that on February 6, 2018, during the announced assessment visit, the children were served pancakes and milk. No fruit or vegetable was served as required. 1240-04-01-.08 FOOD. Corrective Action: A Plan of Corrective Action was left with the provider to complete and return to the Program Evaluator. The PE reviewed with the provider that all required components must be met for each meal and or snack. The provider indicated she understood and would come into immediately compliance. Observation: On February 6, 2018, during the announced assessment, the observed a caregiver repeatedly threaten and place children in time out. Additionally, the assessor observed the Primary Caregiver, repeatedly tell a five year old child to stand at the pool table with his nose against the pool table. The assessor observed the caregiver stand behind the child and did not let him move. The assessor stated the caregiver noted she uses time out with all the children as needed. On February 20, 2018, during the complaint investigation, the caregivers informed the Program Evaluator that the parents of two siblings, ages three and five, requested that when the children misbehave they be made to sit or stand with their nose to the wall or another object. The caregivers noted that this manner of discipline had been regularly used for these two children the past couple of weeks. 1240-04-01-.05 PROGRAM. Corrective Action: A corrective action plan was left with the provider to complete and return to the Program Evaluator. The PE reviewed the rules with the provider on appropriate discipline. PE gave technical assistance on proper discipline techniques. The providers indicated they understood the rules and would come into immediate compliance.
