Observation: A child, age 9 years, fell on the playground and injured her knee. The child exhibited behaviors that may have indicated the injury was serious, however the parents were not contacted immediately following the accident. 1240-04-01-.09 Incident Reporting Corrective Action: The program evaluator provided technical assistance on site regarding deciding when a call to parents is necessary after an injury. The director will be conducting training with all staff regarding proper parent notification of any injury that could potentially be serious in nature.
