See Below for Detailed Information Observation: On 12/20/18, a 5 year old male was pushed off the stage in the gym by a 4th grade female. Three educators were supervising the children but had allowed them on a stage that is not licensed space. None of the educators saw the incident occur. The educators should have been aware of all the children's activity. 1240-04-01-.11 SUPERVISION. Corrective Action: PE discussed with the Director and the educators that space not licensed cannot be used & the importance of supervising children to ensure incidents do not occur. The Director had met with the educators prior to the center opening on 1/8/19 and given a verbal warning to them after reviewing the YMCA's supervision plan. TA was given by PE Judd and a POCA was completed on 1/919. Observations: The gym stage was used for indoor physical play and is not approved licensed space. It is approximately 4 ft. off the gym floor and has items stored that are hazardous. It has not been inspected by and approved for child care by DHS or the State Fire Inspector. 1240-04-01-.16 PHYSICAL FACILITIES. Corrective Action: Two of the educators were aware that the space was not to be used. The Director has discussed the space not being used in a meeting and a verbal warning is going in the educators files. PE has spoken to the educators and the Director separately regarding space not approved for care. A POCA was completed by the Director on 1/9/18 after TA was given by PE Judd.

Observations: Transportation Logs from 6/3 - 6/5/19 were checked prior to PE observing this dates transportation. The monitor on the bus had failed to place her initials on the log after the log off time on all the prior dates as required. A parent had not been required to sign the bus log to show she had received her two children at the bus stop on 6/3/19. 1240-04-01-.17 TRANSPORTATION. Corrective Action: PE did TA with the Co-Directors regarding the transportation logs on site this date. The children were taken home afterwards and the logs were completed correctly. The violation was corrected on site.

Observations: An educator was observed talking harshly to a child and threatening to do a write up on him. This was not appropriate behavior management as required. 1240-04-01-.15 PROGRAM, LANGUAGE, AND LITERACY DEVELOPMENT. Corrective Action: The Director did a plan of corrective action. She will consult with the YMCA and discuss further actions regarding the educator. Trainings will be completed and the issue discussed. The educator will receive a written warning.

Observations; The fire drill was not conducted in November as required. 1240-04-01-.18 EMERGENCY PREPAREDNESS. Corrective Action: The Director completed a plan of corrective action after CCPE gave Technical Assistance. An extra drill will be completed in the month of December since there are but a few days left in November.

Observations: The Director accepted an inhaler from a parent that had expired 2/25/15. 1240-04-01-.12 HEALTH AND SAFETY. Corrective Action: Medications should be in the original container, not expired, and have the child's name on them when received from the parent. The Director with Technical Assistance from Program Evaluator Tammy Judd completed a Plan of Corrective Action. She will meet with the parent today and return the expired inhaler. A new one can be brought when the parent is able. The Director will check medications and create paperwork to ensure that medications are in date.

Observation: A 5 year old child had an inhaler in his backpack. The Director did not know that the rescue medication had to be out of the reach of children. The elementary school had the self administering physicians form but the afterschool program did not. 1240-04-03-.10 HEALTH AND SAFETY. Corrective Action: The backpack was placed out of the reach of the children immediately. The Director will get permission from the parent to help the child with the medication if it is needed at the center. All medications will be out of the reach of the children.

Observations: Several parents had not given written authorization for the center to administer rescue medications if needed. 1240-04-03-.10 HEALTH AND SAFETY. Corrective Action: The Director immediately created a form and immediately emailed the parents whose medications left did not have permission to administer. She explained that the written authorization for the medications would have to be secured today. Later on this date, the Director faxed to DHS Licensing the authorizations the parents signed.
