Observation: The center had unnecessary clutter and was not clean as required. 1240-04-01-.16 PHYSICAL FACILITIES. Corrective Action: PE provided technical assistance and the Director did a Plan Of Corrective Action. The Director will begin cleaning this date. The office, storage, and stage will be added to their daily clean list. The refrigerator will be cleaned monthly.

Observations: Two epi-pens were out of date and had not been returned to the parents. A child at the center is given medicine daily. The parent stated to the YMCA staff the child should receive the medication at 4:30. The physicians directions on the original container state to give the medication at 4:00 pm. 1240-04-03-.10 HEALTH AND SAFETY Corrective Action: PE gave Technical Assistance to the center. A POCA was completed. The out of date medication was returned to the parent and an explanation and education were given. The correct time to give the medication was verified and completed.

Observations: An emergency fire drill had not been completed in March 2018, as required. 1240-04-03-.10 HEALTH AND SAFETY. Compliance Statement: The agency completed a POCA. Two drills will be completed this month to make up for the prior month. After Technical Assistance the Director is aware that fire drills must be completed monthly regardless of other drills being in the same month.

Observation: A medication at the center was without the documentation to show it had been given properly at the correct time, the dosage, any side effects, and the staff person who had given it. 1240-04-03-.10 HEALTH AND SAFETY. Corrective Action: The center completed a POCA and will ensure the documentation is in an area where the person appointed to give it will know where it is located. The office area will be arranged and organized so items will not be lost.
