Observations: A 10 year old male climbed in a tree and fell which resulted in a mild concussion. This occurred without the educators having knowledge of him being in the tree. The level of supervision was not to the level needed and required. 1240-04-01-.11 SUPERVISION. Corrective Action: The Director and PE discussed supervision and offered technical assistance while doing the investigation. The center playground supervision plan needs to reflect children who may need extra attention while on the playground. The educators walk children to meet other educators to go in to leave or to go inside if needed. Communication must be such that the educators left know they have more responsibility. Management will discuss appropriate supervision with all educators. The playground rules will need to include tree climbing is not allowed and educators understand this as well.

Observations: On site was a child's inhaler in a glad sandwich bag. The original container was not on site. The child's name or directions for use were not on the inhaler. 1240-04-03-.10 HEALTH AND SAFETY. Corrective Action: PE discussed the POCA with the Director. The Director will speak to the parent and explain that the medication cannot be kept on site without the original container. She will only accept medication in the original container as required by the licensing rules. PE did a follow up visit on 6/8/18 and checked all medications at the center. All were in compliance. Violation was corrected.

Observations: Greek yogurt set out longer than 15 minutes and was not stored at 41 degrees or below to prevent contamination or spoilage. 1240-04-03-.11 FOOD. Corrective Action: The yogurt was placed in ice placed in a bowl to correct the violation. The Director is going to inquire about using a school refrigerator in the gym. She will purchase a thermometer to ensure the food is kept at 41 degrees or below.

Observations: An inhaler was not in the original container. The name of the child and instructions were not on the inhaler which was in a plastic baggie. 1240-04-03-.10 HEALTH AND SAFETY. Corrective Action: The Director will complete a POCA and discuss with the mother that the medication has to be in the original container always. She will explain that the medication cannot be given due to the agency not being able to ensure the inhaler is for the child it was left at the center for.
