Observation: The 6/28/19 field trip log was not completed as required by a center educator. When the bus arrived at the center after a field trip, the time off the bus and the initials of the educator were not placed on the logs. 1240-04-01-.17 TRANSPORTATION. Corrective Action: PE gave technical assistance prior to the Director filling out the Plan of Corrective Action. The Director will do a refresh training on completing the transportation logs very soon. A Summer Co-Director will complete the logs or check the logs as they are being completed by an educator.

Observations: The mulch on the small playground was not at an acceptable depth. Black felt was showing in two spots near the two climbers. 1240-04-03-.08 EQUIPMENT FOR CHILDREN. Corrective Action: A POCA was completed on the date of the violation. The agency will rake the mulch prior to the children playing. They will check the playground and educators will understand what to look for that is not acceptable.

Observations: A television in the front music room was old and heavy. It was not secured to the portable cart to prevent tip over and possible harm to the children. The strap was lying beside the television instead of on it to secure it. Corrected on site. 1240-04-03-.08 EQUIPMENT FOR CHILDREN. Corrective Action: PE gave technical assistance while observing the music room during the unannounced visit. The agency will review with the educators to make sure they know to check for hazards prior to letting the children do programming in a specific area. The violation was corrected by the strap being placed around the television to secure it to the cart.

Violation

Observations: The Co-Director in charge on 6/12/18 in the morning, accepted cough drops from a parent that were not in the original container as required. 1240-04-03-.10 HEALTH AND SAFETY. Corrective Action: PE provided technical assistance and asked if the co-director had seen emails sent regarding medications that had been sent. PE explained the importance of the directions on the container being followed to protect the well being of the children. The full time Director at the center took the cough drops and placed them in the centers storage room and he will return them to the parent today and explain the rule to the parent. He will discuss the proper procedures and go over the medication rules to further educate. (corrected on site)
