Observation: PE observed a 7 year old leave the homework table and go into the hall without educators being aware. The child was not supervised for under a minute. Her whereabouts were not known as required. 1240-04-01-.11 Supervision Corrective Action: Program Evaluator offered Technical Assistance while informing the Director the child had left and checking on her in the hall. Ideas on managing supervision were offered and the placement of the homework table in relation to where the educators were at the time this occurred. The Plan of Corrective Action was completed by the Director therefore it was corrected on site.

Observation: When PE arrived at the center, the children were outside the cafeteria on the sidewalk and grassy areas having snack in an unfenced area instead of being in the cafeteria using the tables as required. 1240-04-01-.13 FOOD SERVICE. Corrective Action: The children immediately returned to the cafeteria after PE gave Director technical assistance on the dangers of the children being in the unfenced area and having snack on the ground instead of the tables in the cafeteria.

Observation: The kindergarten children were outside the cafeteria on the sidewalk and grassy areas in an unfenced area having snack. Children's outdoor activities must be in areas that have at least 4 ft. fences as required. 1240-04-01-.16 PHYSICAL FACILITIES. Corrective Action: Program Evaluator Tammy Judd provided immediate technical assistance to the Director who had the children return to the cafeteria. It was explained that the hazards of being outside a fenced area are great and endanger the children.

Observations: The Director's purse was on the sign in table which was accessible to the children. Corrected on site. 1240-04-03-.10 HEALTH AND SAFETY Corrective Action: The violation was corrected on site. The director placed her purse in a file cabinet in the closet out of the reach of the children.
