Observations: A serious incident report investigation revealed that children at the center were playing on the playground that does not have enough resilient surfacing as required. Fall zones of swings and climbers must have at least 6 inches of a protective surface. The center management had been aware of this since the center was given a temporary license. The center educators chose to ignore this and allowed children to play on the anchored equipment. 1240-04-01-.14 EQUIPMENT FOR CHILDREN. Corrective Action: PE gave technical assistance. While interviewing the educators PE discussed what the purpose of the rules are and explained it is a safety issue. Also PE discussed the violation with the agency Director. She will ensure that all educators understand the playground supervision plan which states the playground anchored equipment is not used. Ms. Jeudy will update the playground rules to emphasize to the children as well the area cannot be used. Educators will be reminded daily before going out by Ms. Jeudy of the policy. Cones will be used to show the areas that can be used for outdoor play.

Observations: Bleach water was left on the snack cart bottom shelf in the reach of children instead of being placed out of the reach of children as required. 1240-04-01-.12 HEALTH AND SAFETY. Corrective Action: The center educators and the Director should always keep the bleach water used out of the children's reach. This creates a hazard in the center. The bleach water is usually in a cabinet in the Y office. Ms. Jeudy will remind the educators daily to place it up and to not leave on the cart. This was corrected on site this date.

Observation:The meal time supervision plan was not posted in the snack area as required. 1240-04-01-.11 SUPERVISION. Corrective Action: The Director completed a Plan of Corrective Action on 2/11/19. The plan will be posted again. She will speak to the educators regarding not removing and checking to make sure it is on the food cart prior to the children arriving.

Observations: A refrigerator's temperature at the center was 41 degrees and should have been below 40 degrees. 1240-04-01-.13 FOOD AND FOOD SERVICE. Corrective Action: The director completed a plan of corrective action and stated she would check the thermometer daily and adjust it if needed.
