Summary of QRIS Monitoring Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations, complaint investigation, and scoring of the QRIS. Bleach bottles were hanging on the diaper changing table in the Infant 1 classroom, the diaper wipes labeled keep out of reach of children were stored in a cubby and inaccessible to children, but they were not locked in a child-proof cabinet or container. In the Pre-K classroom the bleach water bottle was stored in a high cabinet but it was not locked as required for quality. The steering wheels need to be replaced on the bus on the preschool playground. This is a quality indicator; and not a licensing violation. The children's files were missing some required information such as the parent signature of receiving the licensing rules summary; parent work hours; and emergency contact work hours, work names and work numbers. The lesson plan posted was Moose Tracks and there was no theme based materials observed. During the observation, the children were uninterested in the planned activities. This is a quality indicator and not a licensing violation. During the transition to afternoon snack; the educator continued to stay seated in an adult-size chair and attempted to sing songs. The children were restless and engaged in more active play. The educator continued to sing and did not engage with the children. Most of the language was directive in nature and did not engage the children. The lesson plans posted in the Toddler 1 and Pre-K classrooms did not have TNELDS indicated on the lesson plan. In addition, some of the lesson plans did not include parent involvement. This is a quality indicator; and not a licensing violation. Serve and return conversations were not observed in the Pre-K classroom. This is not a licensing rule; it is a quality standard. The educator did not modify the schedule to support appropriate behaviors. The children in the Pre-K classroom needed a physical activity based on their behaviors after naps. The educator continued to attempt a structured activity. This is a quality standard and not a licensing violation. See Below for Detailed Information During unannounced visits to the agency on April 25, 2022 and April 27, 2022, the agency was cited for twelve (12) violations. Of the twelve (12) violations, nine (9) are considered to place children at a high risk of harm. It is the managements responsibility to ensure that the agency and its staff are in compliance with licensing rules and regulations. (1) Responsibility for Staff. Corrective Action: Observation: When LC arrived at the agency there were 14 children present and only 4 had sign in. Parents started arriving and the were still not signing their children into the agency until asked by the owner/director. 1240-04-01-.11 SUPERVISION.
