The provider will ,provide written proof of acknowledgment and understanding of this rule from the staff to PE. Observation: If reference to the agency's current expulsion policy the provider and educators involved did not follow proper procedure to develop a plan prior to following through with dis-enrollment. 1240-04-01-.15 PROGRAM, LANGUAGE, AND LITERACY DEVELOPMENT. Corrective Action: The provider will develop an additional plan for adjustment behaviors for all children struggling with adjusting to a new environment. The Provider must also review their policies with all staff and provide written proof of understanding of this rule.

A four-year-old child fell in the gymnasium on 4/12/2024 at 11:00 a.m. The child hit his head when he fell while running and tripping over a ball. The educator failed to notify the director immediately. The director and the mother were not notified until 12:36 p.m. The parent was first notified via text; and then with an incident report in Bright Wheel.
