Observation: On 10/29/18, PE's Fain and Garner verified that on 10/26/18 a school age child's time on and off the bus were not documented on the bus log. Additionally, there were no staff initials by the required time off the bus. 1240-04-01-.17 TRANSPORTATION. Corrective Action: A plan of corrective action was completed on site. The agency noted staff would be trained immediately.
