Observation: There were children playing outside in the front part of the playground, but the playground gate was wide open in the back part. If a child had gone around the corner of the building they could have gone out the gate. This placed children at serious risk. Program Evaluator closed the gate this date. A pre-inspection of the playground is supposed to be done before children go outside to play. If the inspection had been done, the open gate would have been seen and would have been closed. 1240-04-01-.16 PHYSICAL FACILITIES Corrective Action: Program Evaluator closed the gate, but also provided technical assistance regarding the seriousness of this violation. A written plan of corrective action was done with the director. The director will review the rules again with all staff and make sure that the playground is fully inspected prior to children going outside to play. Director understood that this could have resulted in a child getting outside the gate.

Observation: Drivers and staff had Department transportation training October 2017, but they did not have it again in April of 2018. It is overdue since this training is required every six (6) months. Verification of the updated training was received on 05/03/2018, as requested. 1240-04-03-.13 TRANSPORTATION Corrective Action: The agency was informed that they could not transport again until the training was completed. The agency drivers and staff had the transportation training that evening, They now meet all requirements necessary to transport children.

Observation: Medicine is kept on a shelf in a closet that is supposed to be locked. Today, the closet was unlocked, making medicine accessible to children. All medicine is to be kept locked up. 1240-04-03-.10 HEALTH AND SAFETY Corrective Action: The closet was locked immediately today. Director will get a box for medicine and keep it locked, with key in a separate place. This will ensure that medication is locked up and inaccessible to children even if the closet is accidentally left unlocked.

There is no statement in the agency policy stating that the center will not release a child to anyone who appears to pose a risk to children.
