Observation: A child, under 2 years of age, was left unsupervised in the facility while the rest of the staff and children went outside to play. The Assessor was with the child during this time. 1240-04-01-.03 STAFF Corrective action: All educators involved will receive mandatory training from CCR&R.

Observation: Thirteen (13) preschool children were present at the group home, this exceeds the allowed capacity of twelve (12). Corrected on site when one child left. 1240-04-01-.21 Group Home Specific Requirements Corrective Action: The primary educator sent one child home for the day so that the capacity was no longer exceeded. The program evaluator reviewed the group home capacity and ratio rules with the primary educator.
