Summary of Unannounced Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint investigation. Technical Assistance Provided: 1. Ensure all educators have training in the Licensing Rules and Regulations before September 30, 2023. All new educators will have the training prior to working with children. 2. Continue to work with Child Care Resource and Referral for on-site technical assistance in the infant classes. (a) To ensure the health and safety of all children enrolled, the management of the child care agency shall maintain a system that enables all children to receive a level of supervision that is appropriate to their age and their developmental, physical and mental status. The child care agency staff shall know where children in their care are at all times, be aware of their activities, and be able to intervene appropriately. Child was left on playground for approximately 3 minutes. Head count was conducted, but not a name-to-face count. Tenn. Comp. R. & Regs. 1240-04-01-.11 SUPERVISION. (3) Playground Supervision. On August 19, 2024, the agency self-reported that a three-year-old child was unattended in the Preschool Red bathroom. It is believed the child returned to the bathroom as his class and another preschool class were transitioning to the playground after lunch. The child was found by another educator who returned him to the playground. The approximate time the child was in the restroom before the educator found him was 1-2 minutes.

Observation: The agency director self-reported that during morning drop off, a 4 year old child ran out of her classroom and out the front doors of the agency to her mother on the sidewalk. 1240-04-01-11 SUPERVISION Plan of Corrective Action: During drop off and pick up, educators will be located near the door and positioned to view door and classroom. However, the department was not made aware of the child being left unattended. 1240-04-01-11 SUPERVISION Plan of Corrective Action: Director states actions were taken in 2016 and continue to present. Educators will call roll by name; count heads at all transitions Parents are asked to follow groups in transition to their destination before signing out and picking up their children. Once children are in line to come in an educator will physically check the tunnel and other 'hiding' spaces.
