Observation: A two month old infant was asleep in a crib on top of a thick blanket and large pillow. There should be no soft bedding or pillows in the crib when an infant is sleeping, This is to help prevent Sudden Infant Death Syndrome (SIDS). Observation: Transportation logs and new employee files were not available for DHS review since the person in charge did not know where they were. DHS needs to have immediate access to all information. Transportation is a high risk activity that needs constant monitoring and DHS needs to make sure children are cared for by qualified individuals. Without access to these items, DHS cannot properly ensure that children are kept safe. 1240-04-03-.05 OWNERSHIP, ORGANIZATION AND ADMINISTRATION Corrective Action: The owner/director will make sure that these files are available for DHS review at all times, even if the owner/director is absent and someone else is in charge.
