Observation: The center has an epinephrine pen for a 15 month old who is allergic to eggs & dairy. The center did not have a physicians action plan that is required for life threatening allergies as required. 1240-04-01-.08 RECORD KEEPING. Corrective Action: TA was given to the Educator in Charge regarding the violation. The Center called the mother concerning an action plan and it was sent to them the center by the Allergy, Asthma, and Sinus the same day which corrected the violation. The center Director completed the POCA and will keep each child's medication forms and action plans on the computer as well as in the classrooms with the children. Medications will be checked weekly.

Observation: The center director reported that an educator was observed by third party witnesses using inappropriate discipline while on the playground. 1240-04-01-.15 Program, Language, and Literacy Development Corrective Action: The incident was self-reported to the department by the center director. The educator involved in the incident was immediately relieved of duty after the agency management was informed by the witnesses. The educator's employment was then terminated.
