Observation: There is no proof that two infants had medical examinations prior to being enrolled at this center. All children under age 60 months are required to have a physical examination prior to being enrolled and the agency is required to have documentation in the records. This ensures that the agency is aware of any medical issues a child may have, and ensures that these medical needs are met. 1240-04-01-.08 RECORD KEEPING Corrective Action: Owner/director states that the children had the physical examinations, but she needs documentation to prove this. Program Evaluator gave technical assistance and stressed the importance of these pre-placement physicals to ensure that all children's needs are fully met. This is especially important for infants and other young non-verbal children. An agency needs to know if an infant has apnea or any other serious medical issues. A plan of corrective action was completed.

The plans help to ensure that children are properly supervised when they eat, to prevent choking. 1240-04-01-.11 SUPERVISION Corrective Action: Program Evaluator provided technical assistance. A Plan of Corrective Action was completed to fix the violation. The agency will make sure it now complies with all new rules. The meal supervision plans will be developed, posted, and followed to help ensure children's safety during mealtime.
