1240-04-01-.09 INCIDENT REPORTING Observation: On September 2, 2020 a child fell while on a field trip and hit their head on a rock which required eight stitches. The agency did not notify the Department the same day of the incident. Incident was reported on September 21, 2020. Plan Of Corrective Action: Agency will be submitting a step by step plan to ensure the department is notified the day of serious incidents.
